Stance Control Systems (SCS) are advanced orthotic technologies used in knee-ankle-foot orthoses (KAFOs) to provide knee stability during standing and walking while allowing the knee to bend more naturally during the swing phase of gait.

Unlike traditional KAFOs, which may keep the knee locked throughout the walking cycle, stance control systems automatically lock or resist knee movement when the leg is bearing weight and release the knee during the swing phase. This can improve walking efficiency, mobility, and safety.

How Do Stance Control Systems Work?

The system detects different phases of walking using mechanical mechanisms, sensors, or microprocessor technology. During stance, when the leg is bearing weight, the knee is stabilized to prevent unwanted buckling. During swing, when the foot is off the ground, the knee is released to allow flexion, making it easier to advance the leg forward.

What are the Main Objectives of Stance Control Systems?

The main objective of stance control systems is to:

  • Improve knee stability during weight-bearing. 
  • Allow more natural knee flexion during swing. 
  • Reduce compensatory movements such as hip hiking and circumduction. 
  • Improve walking pattern and functional mobility. 
  • Potentially reduce the effort associated with walking compared with a continuously locked KAFO. 

What are the Common Types of Stance Control Systems?

The common types of stance control systems include:

  • Mechanical Stance Control Systems: Use ankle movement, foot pressure, or mechanical components to control knee locking and unlocking. 
  • Electronic Stance Control Systems: Use sensors and electronic mechanisms to identify the walking phase and control the knee joint. 
  • Microprocessor-Controlled Systems: Continuously analyze information such as knee angle, movement, and loading to regulate knee resistance and stability. 

Who May Benefit from Stance Control Systems?

Stance control systems may be considered for individuals who require KAFO support due to quadriceps weakness, knee instability, paralysis, or certain neuromuscular conditions, but who have sufficient control and gait ability to safely use a stance-control mechanism. Patient selection and fitting should be performed by an orthotist and rehabilitation team. 

What to Expect During Stance Control Systems Treatment?

During treatment with a stance control system, an orthotist first evaluates your strength, joint stability, walking pattern, and functional needs to determine whether the system is appropriate. The device is then fitted and adjusted to provide knee stability while standing and walking, while allowing controlled knee movement during the swing phase. You may initially need practice with standing, walking, and changing directions under professional supervision. Follow-up appointments help fine-tune the system, address comfort or fit issues, and ensure it is functioning effectively with your gait.
