Morton’s Extensions
What are Morton’s extensions?
A Morton’s extension is a rigid or semi-rigid plate built into an orthosis that runs under the first metatarsal and great toe. It limits upward bending (dorsiflexion) of the first metatarsophalangeal joint. It is different from a Morton’s pad, which is a metatarsal pad used for a different problem (Morton’s neuroma).
Why are Morton’s extensions used?
The extension is used for hallux rigidus or hallux limitus (arthritis or stiffness of the big-toe joint), selected turf-toe injuries, and other conditions in which motion through that joint is painful. By stiffening the first ray, the orthosis reduces painful bending at push-off. Carbon-fiber plates with some flexibility are sometimes used as an alternative when a fully rigid extension is poorly tolerated.
What are the benefits of Morton’s extensions?
- Limits painful motion at the great-toe joint
- May reduce pain with walking and push-off
- Is a common nonsurgical option before considering cheilectomy or fusion
- Can be built into a custom orthosis so the rest of the foot is still supported
