What is diabetic neuropathy?
Diabetic neuropathy is nerve damage related to diabetes. In the feet it often means loss of protective sensation, burning or numbness, and, over time, deformity. Clinics screen for this with monofilament sensory testing: a fine nylon filament is pressed at defined spots on the sole. If the patient cannot feel it, protective sensation is reduced and the foot is at higher risk. Under DME and pedorthotics, the clinic does not treat the nerve disease itself. It identifies that risk, then provides footwear, insoles, and offloading devices that protect insensate skin, plus education on daily inspection.
Why is DME used for diabetic neuropathy?
People who cannot feel a seam, a wrinkle, or a hotspot are at high risk for ulcers and, in some cases, amputation. Monofilament sensory testing is used to diagnose diabetic neuropathy in the feet so that risk is found before a wound appears. Protective shoes, accommodative orthotics, and, when an ulcer is present, offloading boots or similar devices are then used as adjuncts to medical and wound care. They are indicated for at-risk neuropathic feet, not as a standalone cure for numbness.
