Custom LSOs (Lumbar-Sacral Orthoses) and TLSOs (Thoracic-Lumbar-Sacral Orthoses) are prescription spinal braces that are individually designed to fit a patient's body shape and specific spinal condition. Unlike prefabricated braces, custom orthoses are fabricated from detailed body measurements, casts, or 3D scans to provide a precise fit, optimal support, and greater comfort.

What are Custom LSOs (Lumbar-Sacral Orthoses)?

A custom LSO supports the lower back (lumbar spine) and sacral region (base of the spine). It helps limit painful movement, improves spinal alignment, reduces pressure on injured tissues, and promotes healing. Custom LSOs are commonly prescribed for patients with chronic low back pain, lumbar instability, degenerative disc disease, spinal fractures, or after lower back surgery.

What are Custom TLSOs (Thoracic-Lumbar-Sacral Orthoses)?

A custom TLSO extends from the upper back (thoracic spine) down to the lumbar and sacral regions, providing support over a larger portion of the spine. It offers greater stabilization than an LSO and is often used to manage spinal fractures, scoliosis, kyphosis, osteoporosis-related compression fractures, postoperative recovery, and other conditions requiring more extensive spinal immobilization.

What Can You Expect During Custom LSO and TLSO Treatment?

Treatment with a custom LSO or TLSO begins with a thorough evaluation by your physician and orthotist. Your brace will be custom-made using detailed body measurements, a cast, or a 3D scan to ensure an accurate fit and appropriate spinal support.

Once the brace is fabricated, you will attend a fitting appointment where adjustments are made for comfort, alignment, and effectiveness. Your orthotist will teach you how to properly put on, remove, and care for the brace, as well as explain your prescribed wearing schedule.

During treatment, you may need periodic follow-up visits to monitor your healing, assess brace performance, and make any necessary adjustments as your condition improves or your body changes. Many patients also participate in physical therapy to maintain strength, flexibility, and function while the brace supports the spine.
