Custom HKAFO (Hip-Knee-Ankle-Foot Orthoses) are individually designed orthopedic braces that support and stabilize the hip, knee, ankle, and foot. They are prescribed for people who have significant weakness, paralysis, poor muscle control, or instability affecting the lower limbs and require support across multiple joints.

Unlike prefabricated braces, custom HKAFO are made based on the individual’s body measurements, limb alignment, strength, range of motion, and functional needs. They may help maintain proper lower-limb alignment, control unwanted joint movement, and provide stability during standing and walking.

When is Custom HKAFO Recommended?

Custom HKAFO may be recommended for individuals with conditions such as:

  • Spinal cord injuries causing lower-limb weakness or paralysis 
  • Spina bifida 
  • Cerebral palsy with significant lower-extremity involvement 
  • Muscular dystrophy and other neuromuscular disorders 
  • Poliomyelitis or post-polio weakness 
  • Severe lower-limb muscle weakness or joint instability 
  • Certain congenital or orthopedic conditions

What are the Key Components of a Custom HKAFO?

The key components of a custom HKAFO include:

  • Hip Joint Section: Supports and controls hip movement while helping maintain proper pelvic and leg alignment.
  • Thigh Section: Provides support around the thigh and connects the hip joint to the knee joint.
  • Knee Joint Section: Controls knee movement and may lock during standing or allow controlled flexion during walking.
  • Lower-Leg Section: Supports the leg between the knee and ankle while maintaining alignment.
  • Ankle-Foot Section: Stabilizes the ankle and foot and helps position the foot properly during standing and walking.
  • Straps and Closures: Secure the orthosis to the body and help maintain a comfortable, stable fit.

What are the Most Common Types of Custom HKAFO?

The most common types of Custom HKAFO include:

  • Conventional Custom HKAFO: Provides rigid support and stabilization across the hip, knee, ankle, and foot, helping control unwanted movement and maintain alignment. 
  • Reciprocating HKAFO (RGO): Connects the two leg sections through a hip mechanism to assist with alternating leg movement during walking. 
  • Stance-Control HKAFO: Automatically locks the knee during weight-bearing and allows knee flexion during the swing phase, supporting a more natural gait. 
  • Free-Walk HKAFO: Uses specialized knee joints that provide stability during stance while allowing the knee to bend more freely during swing. 
  • Custom Dynamic HKAFO: Incorporates flexible or articulated components to provide support while allowing controlled movement based on the individual's functional needs.

What Can You Expect During Custom HKAFO Treatment?

During Custom HKAFO treatment, an orthotist first evaluates muscle strength, joint movement, alignment, balance, and walking ability. Measurements or a cast/3D scan are then taken to create an orthosis tailored to the individual’s needs. After fabrication, the HKAFO is fitted and adjusted for proper alignment and comfort. Physical therapy and gait training may be recommended to help the patient safely learn how to stand, transfer, and walk with the device. Follow-up appointments are important because adjustments may be needed as the person's condition or functional needs change.
