Custom AFOs (ankle-foot orthoses) are individually designed braces that support and stabilize the ankle and foot while improving alignment, balance, and walking ability. Unlike prefabricated AFOs, custom devices are molded to the patient’s anatomy and tailored to their specific condition, movement pattern, and functional needs.

What are the Common Types of Custom AFOs (Ankle-Foot Orthoses)?

The most common types of custom ankle-foot Orthoses include:

  • Solid AFO: Provides strong support by limiting ankle movement and is useful for significant weakness, instability, or spasticity. 
  • Hinged AFO: Allows controlled ankle movement while restricting excessive or unwanted motion. 
  • Posterior Leaf Spring AFO: Provides flexibility and assists with lifting the foot during walking, commonly for foot drop. 
  • Ground-Reaction AFO: Helps control ankle and knee positioning during weight-bearing and may improve crouched gait. 
  • Carbon-Fiber AFO: Lightweight and energy-efficient, providing support while assisting foot clearance during walking. 
  • Dynamic/Flexible AFO: Offers support while allowing more natural ankle movement for individuals with some preserved muscle control. 
  • Dorsiflexion-Assist AFO: Helps lift the foot during the swing phase of gait and is commonly used for dorsiflexion weakness or foot drop.

How Do Custom AFOs Work?

Custom AFOs typically extend from below the knee to the foot and control unwanted ankle and foot movement. Depending on the design, they may:

  • Help control excessive inward or outward foot movement 
  • Support the foot in a more functional position 
  • Assist with foot drop and improve toe clearance

What Can You Expect During Custom AFO Treatment?

Custom AFO treatment begins with a thorough evaluation of your foot and ankle alignment, muscle strength, range of motion, balance, and walking pattern. The clinician or orthotist also considers your medical condition and functional goals to determine the appropriate AFO design.

Next, measurements of the lower leg, ankle, and foot are taken. A cast, digital scan, or other impression technique may be used to create a precise model for the custom brace. The AFO is then designed and fabricated based on your specific support and movement requirements.

Once the custom AFO is ready, it is carefully fitted to ensure proper comfort, alignment, and pressure distribution. You may be asked to stand and walk while wearing the brace so the clinician can assess its effect on your gait and make any necessary adjustments. Initially, you may need to wear the AFO for shorter periods and gradually increase wearing time as you become accustomed to it. Follow-up appointments may be recommended to monitor your progress, address any discomfort, and adjust the brace as needed.
