CTLSOs (cervical-thoracic-lumbar-sacral orthoses)

Cervical-Thoracic-Lumbar-Sacral Orthoses (CTLSOs) are rigid spinal braces designed to stabilise and immobilise nearly the entire spine, from the neck (cervical spine) to the pelvis (sacrum). They provide maximum external support by restricting movement of the cervical, thoracic, lumbar, and sacral regions, helping protect the spine during healing or while managing complex spinal conditions. CTLSOs are among the most restrictive spinal orthoses and are typically custom-fitted to ensure proper alignment, comfort, and effectiveness.

What Conditions May Require a CTLSO?

A healthcare provider may recommend a CTLSO for:

  • Traumatic fractures involving multiple levels of the spine
  • Cervical and thoracolumbar spinal instability
  • Postoperative protection following complex spinal surgery
  • Severe scoliosis, kyphosis, or other spinal deformities
  • Certain spinal tumours or infections requiring immobilisation
  • Neurological conditions affecting spinal stability

What are the Benefits of a CTLSO?

Benefits of a CTLSO include the following:

  • Provides maximum stabilisation of the entire spine
  • Restricts excessive neck, back, and trunk movement
  • Helps maintain proper spinal alignment during healing
  • Protects surgical repairs and spinal instrumentation
  • May reduce pain by limiting motion at injured spinal segments
  • Can help prevent progression of certain spinal deformities when appropriately prescribed

What are the Common Types of CTLSOs?

CTLSOs are available in several designs depending on the condition being treated, including:

  • Custom-moulded CTLSOs (Minerva-type braces): Fabricated from a mould or scan of the patient's body for precise immobilisation.
  • Prefabricated CTLSOs: Adjustable braces that can be customised to fit an individual patient.
  • Milwaukee Brace: A specialised CTLSO commonly used in selected cases of scoliosis, particularly in growing children and adolescents.

What Can You Expect While Wearing a CTLSO?

Most patients wear a CTLSO for several weeks or months, depending on their diagnosis and healing progress. Your healthcare provider will explain when the brace should be worn, how to put it on safely, skin care recommendations, and when it can be removed for bathing or hygiene. Regular follow-up appointments help ensure the brace continues to fit correctly and provides the required level of support.

Summary

A cervical-thoracic-lumbar-sacral orthosis (CTLSO) is a highly supportive spinal brace that stabilises the spine from the neck to the pelvis. It is commonly prescribed after severe spinal injuries, complex spinal surgery, or for certain spinal deformities to limit movement, maintain proper alignment, and promote safe healing. Available in custom and prefabricated designs, a CTLSO provides maximum spinal protection while helping reduce pain and prevent further injury during recovery.
